1. Insurance Coverage
By registering my child for the School of Sharks Holiday Camp, I acknowledge and understand the following:
- Through Football Federation Australia's (FFA) National Insurance Program, insurance cover is provided to all Gladesville Sharks FC registered players for the 2025 Winter Season.
- If my child is not registered for the 2025 Winter Season with Gladesville Sharks FC, they will not be covered under this policy.
- I accept full responsibility for any medical expenses that may arise in the event of injury or illness during the camp for unregistered players.
- I understand that it is my responsibility to ensure my child has appropriate private health insurance and ambulance cover.
2. Photo & Video Consent
By registering for the School of Sharks Holiday Camp, I acknowledge and agree to the following:
- Photos and video footage may be taken during the camp for promotional purposes by Gladesville Sharks FC.
- This content may be used in future marketing materials, including the club's website, social media platforms, and printed materials.
- If I do not consent to my child being included in photos or videos, I will complete and submit a Photo & Video Opt-Out Waiver to coaching@sharksfc.org.au prior to the start of the camp.
- I understand that while reasonable efforts will be made to exclude my child from group content, complete exclusion cannot be guaranteed in group or action shots.
